Well, Rory (and Amy, I guess) "die" in "Amy's Choice." Rory really dies in "Cold Blood." Amy dies in "The Pandorica Opens." Amy and Rory "die" at the beginning of "Day of the Moon." Rory is drowned in "The Curse of the Black Spot." And Amy sees Rory dead in "The Doctor's Wife."
Only Rory ever dies in reality, but I think that it's more of a meta-joke referencing the show than anything...
Have we ever really gotten an explanation of how being in the Pandorica revived Amy, or how automaton/centurion Rory in the museum in 'The Big Bang' became human!Rory again?
Well, Amy wasn't absolutely and completely dead when she was put in, so the Pandorica kept her in stasis for 2,000 years. When little Amy touched the outside of the Pandorica, it gave the pandorica information so that it could repair big Amy.
Rory is real because the Doctor rebooted the universe, so there is a certain sense in which Rory never became a Roman... and Amy's parents were always alive, etc.
